Application requirements

Before you begin with your application, please be ready with an electronic or scanned copy (in JPEG, JPG, or PNG format and must not exceed 500 kilobytes in size) of the following requirements for upload on this portal. You may use a mobile phone to take a photo of a required document.

• If you are a graduating senior high school student:
• Recent ID picture (2x2, high resolution, white background)
• Certified True Copy of Form 137 from Grade 9 to Grade 11
• Grade 12 Report Card (with complete grades from 1st semester)
• Certification from school stating your rank in Grade 11 class (optional)
• Certificate of Grade Equivalency from the Department of Education (for foreign students)


• If you are a senior high school graduate and has not enrolled in a tertiary school or university:
• Recent ID picture (2x2, high resolution, white background)
• Certified True copy of the Form 137 until Grade 12
• Certificate of Grade Equivalency from the Department of Education (for foreign students)
• Original copy of the grade 12 card reflecting your name and signature of the school officials
• Certification from school stating the honor obtained upon graduation in Grade 12, if applicable


• Additional Requirements for Equity, Diversity and Inclusion (If applicable)
  1. PWD ID for Person with Disability (PWD);
  2. Certificate of Confirmation from the National Commission on Indigenous Peoples (NCIP);
  3. Solo Parent ID issued by CSWD or MSWD if solo parent;
  4. Parent's Solo Parent ID issued by CSWD or MSWD if child of solo parent;
  5. Certification from the Bureau of Jail Management (BJMP) for children of Parents Deprived of Liberty;
  6. Certificate from the Court, the Philippine National Police(PNP), or the Barangay Captain for students affected by domestic abuse or those recognized as victims of a disaster;
  7. Affidavit for children raised by LGBTQ+ parents and by parents who work as caregivers(those working within the country);
  8. Certificate of Employment for working students (household workers);
  9. Latest Income Tax Return of Parents or Certificate of No Income from BIR.
